Complete this form and save it in Word. Return it by mail or email by June 15, 2012. All applicants will be notified by June 20. Scholarships are available; inquire at writersatharriman@yahoo.com. Tuition is $175. Please send $25 with this application. The remaining $150 is payable on acceptance. Details about what to bring, what will be provided, and travel will be included with your acceptance letter.
